Acute promyelocytic leukemia is characterized by atypical promyelocytes that can be hypergranular with “figure eight” nuclei, abundant cytoplasmic granules, and bundles of Auer rods, or hypogranular with similar nuclear shape but indistinct cytoplasmic granules.

In the microgranular variant, the abnormal promyelocyte cells demonstrate a lack of granulation but often have abnormal bi-lobed nuclei. Cells may contain characteristic multiple or bundles of auer rods (light pink) which the cells are then termed “Faggot cells.”

Acute promyelocytic leukemia (APL) falls into the latter category. It is composed of malignant promyelocytes, which often have a distinctive appearance. But the really characteristic finding in APL is the faggot cell, so named because it contains a ton of Auer rods all piled up on each other, resembling a bundle of sticks (or faggot).

We hypothesize a novel origin for Auer body development in promyeloblasts of APL, namely that they originate from peroxidase-positive and expanded rER cisternae, and that primary granules were directly released from these expanded rER elements, bypassing the Golgi apparatus.

Hypergranular APL (75% of cases): large promyelocytes with dense red-purple granulation and variable nuclear shapes/sizes, some cells have > 1 Auer rods. WBC count is usually low. The blood smear may show only a few abnormal promyelocytes.
