分析：从MR上看，肿瘤起源于脑干背侧，髓母细胞瘤可能性较小。故病理性质上多考虑胶质瘤可能性大（弥漫中线胶质瘤，室管膜瘤等）。考虑到该占位已对脑干产生明显压迫，同时导致脑积水，故手术切除为优先选择；替代方案为脑室腹腔分流同时行肿瘤活检。
